For details about how to register to use the EMIS Access facility, please contact our reception.  It will be necessary for you to attend the surgery in person, bringing with you some form of identification such as a driving licence or passport.  If you are unable to attend you must provide another person, acting on your behalf, with signed, written consent and a means of identification with your signature on it to obtain these details.  Please retain the details you are provided with for future reference.
