Please complete this and return it to the surgery reception.

When you have returned the form, please book an appointment to see the nurse for your travel immunisations in one or two weeks time.

Your travel risk assessment form needs to be reviewed by both the nurse and your doctor and we need a minimum of one week to complete these important checks.

The nurse will contact you to inform you if you do not need to attend (i.e. no immunisation is required) or to let you know if any vaccine charges will apply.

Please note that all vaccinations must be paid for at the time of administration.  The practice only accepts cash or cheques made payable to Dr Evans and Partners.  Credit and debit cards are not accepted by the practice.  For a price list of travel immunisations, please click on: Private Charges and scroll to the bottom of the list of charges for non-NHS travel immunisations.
