Combined hormonal contraception: patch, pill, vaginal ring

If the answer to following questions 2-9 is NO and they fulfil criteria in question 1, please issue 12 month prescription of CHC (or if vaginal ring 1 box and 3 month supply on FP10)

If the answer to any of the questions 2- 9 is YES- advise that you are unable to issue them at this time with combined hormonal contraception.

Offer to issue a 12 month prescription for a Progestogen only pill until a time when we can review them to reassess eligibility for a combined hormonal contraceptive.

1. Has normal blood pressure and BMI been recorded in the past year at New Croft or in GP?
2. Have you ever had migraine? These are bad headaches, often only on one side, that make you feel sick or make it hard to look at the light. They may affect ability to see or involve tingling, pins and needles or numbness in the face, arm or leg.
3. Have you had or do you currently have any of the following conditions?

  • High blood pressure
  • Clot in the legs or lungs
  • Stroke
  • Heart disease
  • Breast cancer
  • Any serious illness/operations

4. Have your mother or father had a blood clot in the leg or lungs under the age of 45?
5. Has your mother, father, brother, sister ever had breast cancer?
6. Have you had a baby within the past 6 weeks?
7. Do you have the BRCA gene or a known thrombogenic (blood clotting) mutation?
8. Are you taking any new medications including complementary such as St John’s Wort if so which?

  • If taking an enzyme inducer- recommend injectable, IUD/IUS where feasible.
    In meantime, supply double-dose CHC and advise condoms in conjunction.
  • Teratogenic medication- recommended long acting reversible contraception where feasible.
    In meantime, supply double-dose CHC and advise condoms in conjunction.
